Initial Situation
The patient, a 47 year old female, had previously lost tooth #25 due to recurrent peri-apical infection following multiple attempts to retreat an endodontically treated tooth. Her medical history was characterized by controlled mild periodontal disease she was otherwise medically stable. A residual ridge of 7mm in
hight was visualized with adequate width. After clinical and radiological examination a decision was made
in favour of the placement of a Z-systems Z5c Zirkolith Ceramic Implant (10mm, Ø 4.0mm) and a simultaneous internal sinus lift.

Study:
Osseointegration of Zirconia Implants with Different Surface Characetristics: An Evaluation in Rabbits

Aim:
Histomorphometric assessment of early bone apposition around zirconia dental implants with different surfaces
Methods:
Four times 24 implants – one group with zirconia implants (Z5m, Z-Systems AG, 5th generation with laser modified surface) – were placed in the distal femur of 48 white, female New Zealand rabbits. Half of the implants were harvested at 6 and 12 weeks for light microscopy and histological examination, and the second half was examined for removal torque values.
Results:
The study revealed comparable bone apposition for zirconia and titanium implants at 6 and 12 weeks, and the removal torque values were similar for all implants with rough surface.

Aim:
Case report on a functional and esthetic rehabilitation of a central maxillary incisor
Methods:
The left central maxillary incisor of the patient was endodontically treated and restored after a trauma 14 years ago. Meanwhile, the tooth was considered hopeless due to a fistula as result of an endo-perio lesion. The bony situation allowed implant placement five months after atraumatic extraction and socket preservation. The implant (Z-Look3, Z-Systems AG, 3rd generation with standard sandblasted surface) was manually inserted with a torque value of 20 Ncm, and provisionally restored with a Maryland bridge. After a healing period of four months the final restoration was placed: the abutment was grinded down intra-orally and the crown was cemented.
Discussion:
Z-Look3 one-piece ceramic implants are very well suited for esthetically demanding restorations due to their white color and superior epithelial attachment.

Aim:
Field report on the advantages of zirconia implants


Methods:
Structured discussion on the features and benefits of zirconia as implant material from a dentist’s point of view.


Results:
Zirconia as an implant material has various beneficial features compared to Titanium: Its white color prevents grayish discoloration of the mucosa; even in cases of hard and soft tissue resorption an exposed implant shoulder has a far less negative effect on the esthetic result. Secondly, zirconia has proved to be less susceptible for the adhesion of a pathologic oral microbiota and thus favors long-term gingival health. And lastly, the one-piece design of this implant makes the prosthetic restoration as easy as it is for a natural tooth. Especially given that Zirkolith® implants can be grinded in-situ. However, attention needs to be paid to certain prerequisites: Meticulous planning and a thorough 3 dimensional implant placement are the foundation of long-term clinical success. And special emphasis needs to be put on protective measures to allow for an undisturbed healing of this transgingivally placed implant.
Conclusion: Zirkolith® one-piece implants have esthetic advantages, especially in anatomically demanding situations.

Immediate occlusal vs. non-occlusal loading of single zirconia implants.
A multi-center pragmatic randomised clinical trial

Aim:
Comparison of immediate non-occlusal loading vs. immediate occlusal loading of single zirconia implants with regards to early failures


Methods:
40 implants (Z-Look3, Z-Systems AG, 3rd generation with standard sandblasted surface) were placed in 40 patients with the aim to compare two loading protocols. All implants were inserted with a torque of at least 35 Ncm, provisionalized with acrylic crowns at the same day, and definitively restored after 4 to 5 months.


Results:
At one year, the survival rate was 87.5% with 5 early implant losses, three in the occlusal and two in the non-occlusal group. There was no statistically significant difference between the groups. But there was a correlation between time of implant placement and implant failure: 4 out 10 implants in extraction sockets failed, compared to only 1 out 30 implants with delayed placement.
Conclusion: The results of this study do not provide a conclusive answer.

Study:
Quality of the peri-implant soft tissue attachment of zirconia implants (-abutments): Comparison of the results of a literature review with the experiences of dental practitioners

Aim:
Literature and Clinical evaluation of zirconia implants and abutments regarding soft tissue attachment
Methods:
A literature review resulted in 16 relevant publications evaluating soft tissue attachment, plaque adhesion, bacterial colonization with germs associated with periodontal diseases, inflammation factors of the soft tissue, as well as the influence on the color of the peri-implant soft tissue. And in the clinical part, 65 zirconia implants (Z-Look3, Z-Systems AG, 3rd generation with standard sandblasted surface) were placed in 34 patients following a protocol that places emphasis on protective measures. The implants were examined annually with a mean time of approximately 22 months in-situ.
Results:
Compared with titanium, the zirconia implants and abutments achieved comparable or better results in all studies. At all times in these investigations, the evaluation of the clinical findings showed average probing depths between 2 and 3 mm. Plaque and bleeding results were described as exceptionally good.
